Gather Required Documents

To complete enrollment, please include the following:

  • Physical Exam (valid for 3 months)

    Prospective participants can submit a summary of physical exam generated by their physician or use our center form.

  • TB Clearance (within the last 12 months)

    TB clearance is not required for authorization but is required prior to program attendance in compliance with CDPH regulations.

  • Current Medication List
  • Copy of Medi-Cal and insurance card
  • ID (Driver's License / State ID)
  • Social Security card
  • Medicare card (if dual eligible)
  • Emergency contact information
