To complete enrollment, please include the following:
- Physical Exam (valid for 3 months)
Prospective participants can submit a summary of physical exam generated by their physician or use our center form.
- TB Clearance (within the last 12 months)
TB clearance is not required for authorization but is required prior to program attendance in compliance with CDPH regulations.
- Current Medication List
- Copy of Medi-Cal and insurance card
- ID (Driver's License / State ID)
- Social Security card
- Medicare card (if dual eligible)
- Emergency contact information
